Transitional Care Leadership Summit

Project Summary: 

Health Research for Action convened a meeting of national stakeholders to discuss transitional care innovations recommended in our strategic study, "From Hospital to Home." More than 50 participants attended. They represented health care systems, professional associations, research institutions, transitional care programs, foundations, policy and advocacy organizations, the Centers for Medicare and Medicaid Services, the US Administration on Aging, and the California Health and Human Services Agency. The summit identified many important actions to improve transitional care through educational interventions, community supports, provider training, and system- and policy-level changes.

Policy, Practice or Research Impacts: 

This summit was based on one of the most comprehensive assessments to date of issues related to hospital-to-home transitions for seniors. The summit identified many important actions to improve transitional care through educational interventions, community supports, provider training, and system- and policy-level changes.
