HRA developed the Sample Health Plan Application to provide health plans with language, designs, and ideas to create application forms that people can easily read, understand, and use. The Sample Application is based on best practices in health literacy, and formative research and evaluation. It was developed in English, Spanish, and Chinese for large-group and small-group full service Health Care Service Plans licensed by the Department of Managed Health Care in California.
